So a few of my players have now gotten insanities and are starting to get annoyed with them and are exploring options for removing them.

The book talks about surgery and the Shallyans have their ways as well as the lore of metal, but it doesn't really talk about the compensation a character would have to spend to gain these services. Any suggestions? I would think the surgeon being riskier and easier to find would be cheaper than the other two options, but not sure on how to price something like that.

Much of this depends on how your players have played. Karma is a bitch.
If they are all thugs, then the Shallya option should be non-existant unless you do something like have them take a vow of non-violence. If the players try to do anything violent, it makes them sick. Use 'A Clockwork Orange'. If you have not seen it...Go see it!

The doctor route is likely to have the players cured of one insanity, only to gain another.
Do not tell the players how the gamemechanics will work, but here is a rough guideline.
We will use 100Gold as the baseline, but the more the characters spend, the better their chances of getting a cure. I like something like this because the players only screw themselves when they try to be cheap.

01-10 or 1% for every 10 Gold spent: Cured
11-30 or next 20% Cured..sort of : Remove Insanity, but add 6 insanity points
31-50 or next 20% Something New: Replace the insanity with a different one
51-70 or Next 20% Nothing
71-80 or next 10% Near death experience gain D6 Insanity points
71-90 or next 10% Boutched - Gain one additional insanity
91-00 remaining: Brain damage: Spend a fate point or lose 3D10 from INT

The spell in the lore of metal removes insanity points, not insanities.

For other magical cures, create some similar to the doctor table.

Alternitively, you could be nicer.....but this is Warhammer.
